Payment, if required, in the form of a check made out to “Menomonie Alliance Church” or cash can be either mailed (to the address above) or dropped off at the church office during regularly scheduled business hours (8:30am-4:00pm M-F). If a key fob is needed for entrance to the building for your event, you may pick it up in the office the week of your event and after payment is made.

Your group is responsible for setting up the space (except for Class A events). Tables and chairs will be in the room or in the hallway outside the room. When your event is over, please leave the space as you found it, wipe off and take down the tables, and stack the chairs. If needed, dumpsters are located on the north side of the building.


If yes, Pastor Wes will be contacting you to schedule a training session to take place at least seven days before your scheduled event.

If yes, we require a training session with the lead of the coffee ministry. The lead of the coffee ministry will be contacting you to schedule a training session to take place at least seven days before your scheduled event. Any ministry of the church will be able to use the supplies in the coffee area. Any community/outside group will be asked to bring in their own supplies (coffee, creamers, sugar, stir sticks, cups, etc.).
We expect the facility and equipment to be treated with care and respect. By submitting this form you agree to:

  1. Refrain from using alcohol and/or illegal drugs in the building or on the grounds.
  2. Report any damage, accidental or otherwise, to the church office promptly.
  3. Keep to the area(s) you reserved to accommodate everyone using the church facility.
  4. Always provide supervision to all children.
  5. Help us not waste electricity by turning off lights if you are the last one in an area.
  6. Make sure the outside doors are locked when you leave if you're the last one in the building.
  7. Events held on a Saturday must be done by 8:00pm for staff to prepare for Sunday morning.
  8. Communicate any changes to the church office as soon as possible. I understand that the changes may not be able to be made.


A signature below indicates:
  • I agree that Menomonie Alliance Church can cancel my event if it will use the church premises for any purpose that is contrary to the Menomonie Alliance Church Christian Community Policy, mission, purpose or belief of the church, which is a biblically-based religious institution.
  • I agree to hold harmless, indemnify and defend the church (including the church’s agents, employees, and representatives) from any and all liability for injury or illness that may result from our group using church property. I take full responsibility for myself and all attendees if they become injured or ill following our event at Menomonie Alliance Church.
