Facility Use Agreement

We expect the facility and equipment to be treated with care and respect. By submitting this form you agree to:

  • Refrain from using alcohol and/or illegal drugs in the building or on the grounds.
  • Report any damage, accidental or otherwise, to the church office promptly.
  • Keep to the area(s) we reserved to accommodate everyone using the church facility.
  • Always provide supervision to all children.
  • Help us not waste electricity by turning off lights if you are the last one in an area.
  • Make sure the outside doors are locked when you leave if you’re the last one in the building.
  • Events held on a Saturday must be done by 8:00pm for staff to prepare for Sunday morning.
  • Communicate any changes to the church office as soon as possible. I understand that changes may not be able to be made.
  • Menomonie Alliance Church can cancel this agreement if it will use the church premises for any purpose that is contrary to the Menomonie Alliance Church Christian Community Policy, mission, purpose or belief of the church, which is a biblically-based religious institution. We may also cancel our agreement by having a discussion with the Scheduling Team.
  • Hold harmless, indemnify and defend the church (including the church’s agents, employees, and representatives) from any and all liability for injury or illness that may result from our group using church property. I take full responsibility for myself and all attendees if they become injured or ill following our event at Menomonie Alliance Church.
I agree to the terms stated above. These terms apply any time the below mentioned group/individual uses Menomonie Alliance Church in 2024 & 2025.
